Physical Therapist (PT)

Schedule / Job TypeContract | Days | Monday–Friday | 36 hours per week minimum
Key Responsibilities- Provide direct physical therapy services to patients across assigned facility locations
- Evaluate patients and develop individualized treatment plans to address functional deficits
- Deliver therapeutic interventions in accordance with physician orders and plan of care
- Document patient progress, treatment notes, and discharge planning in a timely manner
- Collaborate with interdisciplinary care teams to coordinate patient care
- Maintain compliance with facility policies and applicable regulatory standards
- Current and active Physical Therapist (PT) license in the state of Montana
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
- Ability to fulfill a 12-week contract assignment
- Prior experience in a skilled nursing, rehabilitation, or long-term care setting
- Experience working across multiple care sites or split-facility assignments
- Opportunity to make an immediate impact providing coverage in a supportive rehabilitation environment
- Consistent Monday–Friday daytime schedule with no weekend requirements
