$2400/week
The Physical Therapist will provide patient-centered therapy services in Clearfield, PA, supporting outpatient rehabilitation and campus-based care needs. The Physical Therapist will work primarily at the Outpatient Rehabilitation Center and may float as needed across Med Surg, Emergency Department, and Geriatric Behavioral Health areas. This role will evaluate patients, develop individualized plans of care, deliver skilled interventions, document progress, and collaborate with interdisciplinary teams to support functional recovery, safety, and discharge planning.
Physical Therapist Responsibilities:
- Evaluate adult and geriatric patients, assess functional status, identify barriers to mobility and recovery, and establish individualized short- and long-term therapy goals.
- Develop and implement plans of care using therapeutic exercise, gait training, mobility training, strengthening, range-of-motion activities, pain assessment, body mechanics education, and appropriate modalities.
- Provide care for patients with orthopedic, neurologic, cardiopulmonary, deconditioning, fall-related, post-surgical, and geriatric behavioral health needs, including diagnoses such as TKA, THA, TSA, CHF, Parkinson’s disease, dementia, and mood disorders.
- Document evaluations, reassessments, discharge plans, goals, and treatment progress accurately in Cerner while managing a daily caseload of approximately 10 visits.
- Collaborate with Physical Therapy, Social Services, Rapid Response Teams, providers, patients, and families to support safe care delivery, patient education, injury prevention, discharge planning, and interdisciplinary coordination.
Physical Therapist Qualifications:
- Active Physical Therapist licensure and ability to practice in Pennsylvania is required.
- BLS certification is required.
- Experience in outpatient clinic, acute care, orthopedic, geriatric, neurologic, or rehabilitation settings is preferred.
- Cerner charting experience, teaching hospital experience, community hospital experience, and experience with 1:1 patient care are preferred.
- First-time travelers are welcome; PA fingerprinting is required prior to start.
