Job Summary
Physical Therapist Home Health in the designated territory. This position involves providing skilled physical therapy services to geriatric patients within a home health setting, adhering to clinical standards and documentation requirements. The role offers an opportunity to deliver impactful patient care in a flexible and dynamic environment.
Job Details
- Certifications, Education Requirements, Degree Requirements, Location Requirements, and Years of Experience: Valid license in hand, minimum of 1 year of relevant experience, and current BLS certification.
- Schedule: Day shift, 8-hour shifts, minimum of 40 hours per week.
- Required Qualifications: Valid license in hand, active drivers license, minimum of 1 year relevant experience, proficiency in HCHB (Homecare Homebase), OASIS experience required for staff completing Start of Care (SOC), timely communication and documentation mandatory, Monday orientation.
- Additional Requirements: All travelers are expected to be flexible with territory, and all charting must be completed by 9 a.m. the following morning.
- Responsibilities: Deliver skilled physical therapy to geriatric patients, utilizing appropriate techniques and equipment, ensuring patient progress and safety, and adhering to the organizations clinical protocols.
- Submission Requirements: Active license in hand, personal transportation, RTO (Requested Time Off) must be submitted at application, and on-call rotation may be required.
