Job Summary:
Home Health Physical Therapist (PHYSICAL THERAPIST HOME HEALTH) in Santa Rosa, California. This position involves providing rehabilitative care to geriatric patients in a home health setting. The role requires flexibility with territory and adherence to clinical documentation and communication standards.
Job Details:
- Schedule: 8-hour shifts, minimum of 40 hours per week.
- Required Qualifications: Valid license in hand, active driver's license and personal transportation, minimum of 1 year of relevant experience, proficiency in Homecare Homebase (HCHB) for staff completing Start of Care (SOC) visits, current Basic Life Support (BLS) certification, and experience with patient charting and documentation.
- Preferred Qualifications: Experience with OASIS in home health care is required for SOC visits.
- Additional Information:
- All charting must be completed by 9 a.m. the following morning.
- Timely communication and documentation are mandatory.
- On-call rotation may be required; to be discussed during interview.
- The position involves working with geriatric patients, managing a typical patient load of 5-7 visits per day.
- No first-time travelers accepted.
- Orientation scheduled for Monday.
- All travelers must have an active drivers license and personal transportation.
- RTO (Requested Time Off) must be submitted at the time of application.
- Modular billing available; modules are completed during orientation.
- Travelers are expected to be flexible with territory within a 60-mile radius of the hub/office.
