Job Details:

- Required Qualifications: Vermont Physical Therapist licensure; Home Health experience and OASIS experience are essential, as this candidate will case manage; McKesson/Netsmart Electronic Medical Record (EMR) experience is strongly preferred; no requested time off.
- Preferred Qualifications: Experience with therapeutic measurement techniques including Range of Motion (ROM), strength, and balance tests; ability to utilize various therapeutic modalities.
- Additional Information: The position emphasizes collaborative patient assessments and treatment planning to maximize patient outcomes and independence.
