Job Summary
Occupational Therapist (OT) - Home Health - Medford, OR and Surrounding Areas. This position involves providing occupational therapy services to patients in a home health setting. The role requires delivering care according to established clinical standards, maintaining documentation in charting systems, and working independently within a community-based environment.

Job Details

  • Position Description: Home Health OT working in Medford, OR, and surrounding areas, with an immediate start date. The shift is 8 hours during the day, with a minimum of 40 hours per week.
  • Required Qualifications: Certification in Basic Cardiac Life Support (BCLS); at least 1 year of skilled home health experience; valid State license (including compact license); proven charting system experience; minimum 1 year of work experience; COVID-19 vaccination (primary series and booster) required; must have own transportation.
  • Schedule: Dayshift, 8 hours per day, minimum of 40 hours weekly.
  • Additional Information: The role expects the therapist to work independently with a patient ratio of 1:1, visiting up to 6 patients per day. Occasional maximum radius of 50+ miles may apply. Candidates must commit to working 3 of the 5 designated holidays and request all RTO or specific scheduling requests at time of application. Certification in BCLS is required, and Oasis experience is preferred. Candidates should be prepared to work in a community home health setting, providing care that includes activities of daily living, biofeedback, cardiac rehabilitation, edema massage, electrical stimulation, and various splinting and prosthetic techniques.
  • Submission Requirements: Candidates must include responses regarding prior experience at a Providence facility, references covering 1 year (6 months for CNAs) within the last 3 years, and detailed scheduling preferences.
