SUMMARY

The Outreach & Enrollment Coordinator is responsible for engaging prospective and current members, supporting enrollment into Enhanced Care Management (ECM) and other available services, and connecting members to appropriate healthcare and community resources. This position conducts telephonic and in-person outreach, participates in community events, coordinates referrals, and ensures timely and accurate documentation in accordance with organizational and program requirements. The ideal candidate is a strong communicator who is comfortable engaging with diverse communities, working independently in the field, and collaborating with internal teams to help members access the care and services they need.


WHERE YOU’LL WORK

This position is hybrid. Work from home and go into the field (Sacramento) as needed. You are eligible for mileage reimbursement for the use of your vehicle for business-related travel. Standard business hours are Monday through Friday from 8:30 am to 5:00 pm. 


WHAT YOU’LL DO

Outreach & Enrollment

  • Implement outreach strategies to identify, engage, educate, and enroll eligible prospective members in Enhanced Care Management (ECM) and other applicable programs
  • Verify member eligibility and program criteria to support appropriate enrollment
  • Conduct telephonic, in-person, and other outreach activities to engage prospective and eligible members
  • Complete enrollment intakes, assessments, and required documentation accurately and within established timelines
  • Obtain and document required member consent and enrollment information
  • Upload required enrollment forms and supporting documentation to the member's electronic health record (EHR)
  • Conduct follow-up outreach and coordinate referrals to support successful enrollment and connection to services

Community Outreach & Representation

  • Represent Titanium Healthcare in a professional and culturally responsive manner at community events, health fairs, meetings, resource fairs, and other outreach activities
  • Organize, coordinate, and participate in community health events, informational sessions, and outreach campaigns to increase awareness of available healthcare and social services
  • Conduct in-person outreach, including community canvassing, field outreach, and other assigned outreach activities
  • Build positive relationships with community members, organizations, healthcare partners, and community-based organizations
  • Distribute educational and program materials and provide information about available services and resources

Member Services & Care Coordination

  • Identify, coordinate, and follow up on member referrals to promote continuity of care and connection to appropriate services
  • Collaborate with Lead Care Managers (LCMs) and other internal teams regarding member enrollment, progress, care needs, and service coordination
  • Support warm handoffs and connections between members and healthcare, behavioral health, and community-based services
  • Assist members in navigating available resources and addressing barriers to accessing care and supportive services
  • Maintain a member-centered, trauma-informed, and culturally responsive approach in all interactions

Documentation, Quality, & Compliance

  • Document all outreach, enrollment activities, member interactions, and enrollment evidence in the EHR accurately, concisely, and within required timeframes
  • Maintain accurate and complete member records in accordance with program, contractual, and organizational requirements
  • Protect member confidentiality and adhere to all applicable privacy and compliance requirements
  • Follow Titanium Healthcare policies, procedures, safety protocols, and injury prevention guidelines
  • Meet established outreach, enrollment, productivity, documentation, and quality performance expectations
  • Perform other duties and projects assigned

Sensory Requirements

  • Fluent in English (written and verbal), Bilingual in Spanish or Vietnamese 
  • Ability to communicate clearly in-person, by phone, and electronically
  • Adequate hearing and vision (with corrective devices if necessary) to conduct assessments and documentation
  • Ability to identify problems and use logic and related information to develop and implement solutions
  • Excellent organizational skills and attention to detail
  • Commitment to maintaining patient confidentiality and adhering to ethical standards in healthcare practice

Physical Activity

  • Ability to lift, carry, push, or pull up to 20–25 pounds (e.g., laptop bag, forms, mobile equipment)
  • Ability to climb stairs or navigate uneven terrain in community and home environments
  • Ability to bend, reach, and conduct in-person visits in non-traditional environments
  • Must be able to remain in a stationary position
  • Must be able to move around the office and/or travel throughout community
  • Ability to operate a vehicle and travel to meet with members around the community; attend meetings and events as required or requested

Environmental Conditions 

  • Work may occur in homes, shelters, outdoor settings, hospitals, or community organizations, which may include exposure to pets, smoke, odors, clutter or unsanitary condition, and varying temperature conditions
  • Ability to maintain professionalism and safety in diverse environments
  • Ability to work independently and carry out assignments to completion within the parameters of established policies and procedures

Technology Use

  • Frequent use of computers, keyboard, and handheld/mobile devices
  • Ability to type for extended periods
  • Competent with computers, email, virtual platforms, and Microsoft Office based programs


WHAT YOU’LL NEED

  • Diploma/GED required, associate degree a plus 
  • Minimum 1+ years of experience in outbound call center operations, community outreach, canvassing, enrollment, or member engagement
  • Minimum 1+ year of experience in healthcare, social services, managed care, community-based organizations, or another related field
  • Current and valid Driver’s License with a clean driving record 
  • Reliable transportation and proof of auto insurance
  • Distraction-free home workspace with a secure internet connection

  • Experience working with Medi-Cal, CalAIM, Enhanced Care Management (ECM), Community Supports (CS), or other healthcare and social service programs
  • Experience conducting field-based outreach or engaging hard-to-reach or underserved populations
