Job Description & Requirements

2 years CURRENT PT experience [Required] (New grads will not be considered for this need)
DOCTORATE [REQUIRED]
Updated CAQH must be provided after start
NPI must be provided after start
Must be flexible - Will need to work Inpatient PT as needed
Must be able to treat all normal diagnoses seen in outpatient clinics.
Graduate from an accredited school of Physical Therapy - Required
Previous Rehab Experience - Required
Previous travel experience [strongly preferred]
CPSI EMR experience - [Highly Preferred]
Previous experience working with PTA's - [Preferred]
Traveler perm address must be included in profile
Certification Requirements
BLS (AHA)
State License Requirements
Vermont
Additional License Requirements
Facility will pend license upon offer. Must apply within 24hrs of offer acceptance
