As a Home Health Physical Therapist in this area, you will provide one-on-one, evidence-based care to patients in their homes across Paris and surrounding communities. You will play a key role in helping patients regain mobility, independence, and quality of life after illness, surgery, or hospitalization. Typical patients include adults and seniors with orthopedic conditions, neurologic conditions such as stroke, cardiopulmonary diagnoses, balance and gait issues, chronic disease management needs, and general deconditioning. Your day typically includes travel between patient homes within an assigned territory, performing comprehensive assessments, developing individualized plans of care, and implementing interventions such as therapeutic exercise, gait training, balance and fall prevention, home safety assessments, and caregiver education. You will complete initial assessments, routine visits, reassessments, and discharge visits in accordance with regulatory and agency standards. Depending on the agency’s structure and your experience, you may complete OASIS assessments and collaborate closely with intake, nursing, and case management teams. Workload and patient volume are designed to support quality care; you can expect a reasonable number of visits per day, depending on geography and scheduling, with time allotted for documentation and communication. Because care is delivered in patients’ homes, there are no traditional “beds” to manage, but you will be responsible for managing a defined caseload within your territory. You will work closely with nurses, occupational therapists, speech therapists, and other clinicians to coordinate comprehensive, patient-centered care. Typical schedules may include Monday through Friday daytime hours, with some flexibility to meet patient needs; specific scheduling expectations may vary by agency and territory. There may be occasional weekend or on-call responsibilities, but these will be clearly communicated by the hiring organization. The role emphasizes autonomy and independence while still offering strong support from clinical leaders, office staff, and an interdisciplinary team that is only a call or message away.
