13-week Physical Therapist (PT) assignment in a Skilled Nursing Facility (SNF), working 5x8-hour day shifts (8:00 AM–4:30 PM). This position offers a 35-hour guaranteed workweek and focuses on evaluating and treating patients with orthopedic, neurological, and medically complex conditions. The Physical Therapist will develop individualized treatment plans to improve mobility, restore function, and maximize each patient's independence.
Requirements:
Active Physical Therapist (PT) license in the applicable state
Minimum 2 years of Skilled Nursing Facility (SNF) experience preferred
Current BLS certification
Strong evaluation, treatment planning, and documentation skills
Experience with gait training, balance, strengthening, and mobility interventions
Knowledge of Medicare documentation and rehabilitation standards
Excellent communication, organization, and interdisciplinary collaboration skills
