Palliative Care Manager (LMSW)

Position: Palliative Care Manager (LMSW) – Full-Time
Location: Grand Rapids, MI 
Shift: Monday–Friday - 8:30 AM – 5:00 PM
Salary: $75,000 – $85,000 annually

Responsibilities:

• Lead the startup and implementation of the palliative care program
• Develop workflows, policies, and operational processes
• Manage program budget and financial performance
• Supervise and support palliative care staff in collaboration with the Medical Director
• Ensure compliance with Medicare, Medicaid, commercial payer, and accreditation requirements
• Assist with provider credentialing and payer contracting
• Oversee referrals, scheduling, documentation, and care transitions
• Drive program growth, quality improvement, and exceptional patient service
• Provide direct social work services during program startup and as needed
• Participate in the hospice Manager On-Call rotation

Qualifications:

• Master's Degree in Social Work (MSW) required
• Licensed Master Social Worker (LMSW) preferred/required per state regulations
• Leadership experience in hospice or palliative care preferred
• Program development experience preferred
• Experience with Electronic Medical Records (EMR) systems
• Proficiency with Microsoft Office
• Excellent leadership, communication, organizational, and collaboration skills
