Job Description
Job Summary
We are seeking experienced and motivated Assessors to complete Clinical Behavioral Health Assessments (BHA) for children, adolescents, and adults in our Non‑Intensive Outpatient Program (NIOP). Assessments may be completed in‑clinic, virtually, or in the community.
This position involves evaluating mental health and/or substance use needs, diagnosing, and developing treatment plans in alignment with DBHDD and CMO guidelines.
Minimum Qualifications:
Supervisee / Trainee
Master’s degree in Mental Health, Community Counseling, Psychology, Social Work, or a related field (at time of interview)
Actively enrolled in practicum/internship
Under clinical supervision and working toward licensure
Associate Licensed Clinicians
LMSW, APC, AMFT
Under clinical supervision and progressing toward LPC, LCSW, or LMFT
Valid, active license required
Fully Licensed Clinicians
LCSW, LPC, LMFT
Valid, unrestricted license required

Key Responsibilities
- Complete Clinical Behavioral Health Assessments (BHA)
- Conduct client and/or caregiver interviews
- Collect and document collateral information
- Administer and interpret required clinical tools, including: Safety Plan
Discharge Plan
C‑SSRS
LOCUS (Adult) / CALOCUS (Child & Adolescent)
- Complete Substance Abuse Assessment (if needed)
- Obtain client or guardian signature pages (original or approved e‑signature)
- Enter Treatment Plan and Diagnoses in Sharenote
- Complete Order of Service
- Submit all required written reports
