Faith Healthcare Services is seeking compassionate and dedicated Physical Therapists (PTs) and Occupational Therapists (OTs) to join our growing home health team. If you're passionate about helping patients improve their mobility, independence, and quality of life, we'd love to hear from you!

We have immediate opportunities for therapists to provide one-on-one skilled therapy services to patients in their homes throughout Palm Beach County, including West Palm Beach, Boca Raton, Delray Beach, Boynton Beach, Lake Worth, Wellington, Royal Palm Beach, Greenacres, Palm Beach Gardens, Jupiter, and surrounding communities.

• Perform comprehensive patient evaluations

• Develop individualized treatment plans

• Provide skilled physical or occupational therapy services in the home

• Educate patients and caregivers on treatment plans and home exercise programs

• Accurately document patient progress and treatment outcomes

• Collaborate with physicians, nurses, and the interdisciplinary care team to ensure exceptional patient care

• Active Florida Physical Therapist (PT) or Occupational Therapist (OT) license

• Home health experience preferred

• Strong clinical assessment and documentation skills

• Excellent communication and patient education abilities
