About the Role:
As a Physical Therapist (PT) at Uniontown Nursing and Rehab, you will be responsible for providing high-quality physical therapy services to our residents. This includes assessing patient needs, developing and implementing treatment plans, and evaluating patient progress. You will work closely with our interdisciplinary team of healthcare professionals to ensure that our residents receive comprehensive care that meets their unique needs.
Responsibilities:
Conduct initial and ongoing assessments of patients' physical therapy needs
Develop and implement individualized treatment plans to achieve patient goals
Provide physical therapy interventions, including exercises, manual therapy, and education
Evaluate patient progress and adjust treatment plans as necessary
Communicate with patients, families, and healthcare team members to ensure seamless care
Maintain accurate and detailed patient records
Requirements:
Per Diem Physical Therapist (PT) certification with a degree from an accredited Physical Therapy program
Minimum of 1 year of experience as a Physical Therapist
