What You Bring:

• Master's degree in Social Work (MSW) or Licensed as a Clinical Social Worker (LCSW) from an accredited school

• 2-3 years of experience required, with strong community knowledge

• 1+ year of home care experience preferred

• Ability to exercise initiative, sound judgement, and independent decision-making

• Strong collaboration and communication skills
