Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans and goals aimed at progressing patients toward discharge within an established length of stay.
• Collaborate with the interdisciplinary team to address all barriers to discharge, including equipment trialing and ordering, patient and family training, home exercise program issuance, and planning for continuing services.
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and intermediary guidelines.
• Effectively manage individual, concurrent, and group treatment sessions based on patient care needs, demonstrating flexibility in all aspects of caseload management.
• Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
• Actively engage in professional development through continuing education, self-study, and involvement in professional activities and organizations.

Required Qualifications

• Active Physical Therapist license in the state of Florida.
• Current Basic Life Support (BLS) certification.
• Proficiency in EPIC electronic medical records (EMR) system.
• Prior experience in acute care and inpatient rehabilitation settings.
• Adherence to discipline-specific State Practice Act and all applicable regulatory agency requirements.

Preferred Experience / Skills

• Experience with safe patient handling practices and workplace safety protocols.
• Familiarity with performance improvement methodologies and quality improvement initiatives.
• Experience participating in patient and professional advocacy efforts, including community-level events or professional memberships.
