Presidential Home Care LLC is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our growing team in Woodstock, GA. Responsibilities:

• Provide direct patient care including wound care, medication administration, and vital sign monitoring
• Develop and follow individualized patient care plans in coordination with supervising RNs and physicians
• Observe and document patient conditions, reporting any changes to the appropriate healthcare team members
• Assist patients with activities of daily living (ADLs) and personal hygiene as needed
• Educate patients and family members on care procedures, medications, and health management
• Maintain accurate and timely clinical documentation and medical records
• Coordinate with home care team members to ensure continuity of care

Requirements:

• Active and unrestricted LPN license in the state of Georgia
• Previous home health, long-term care, or clinical nursing experience preferred
• Strong knowledge of nursing procedures, medication protocols, and patient assessment
• Excellent communication and interpersonal skills with patients and families
• Ability to work independently and manage time effectively in a home care setting
• Valid driver's license and reliable transportation required
• CPR/BLS certification preferred
