You'll collaborate closely with consumers, caregivers, and community partners to ensure individuals receive the care and resources they need-while empowering them to stay in control of their care.Why This Role Might Be Perfect for YouThis position is ideal for nurses who are ready to step away from traditional bedside care but still want to make a meaningful impact.No weekends or overnight shiftsFlexible scheduling to support work-life balanceLess direct patient care, more coordination and advocacyUse your clinical expertise in a new, rewarding wayBuild meaningful, long-term relationships with those you supportYou'll continue to make a difference every day-just in a more sustainable, flexible, and empowering way for both you and the people you serve.What You'll DoCare Planning & CoordinationConduct assessments to determine eligibility for PCA and other in-home support servicesDevelop, review, and adjust individualized care plans to ensure evolving needs are metCoordinate services and connect consumers to community resources that enhance independenceConsumer & Caregiver SupportProvide training and education to caregivers (ACEs) to ensure success in their rolesEmpower consumers through skills training focused on independence and self-directionSupport hiring processes for care attendants, including application guidance and onboardingMonitoring & AdvocacyMonitor care plans quarterly (or more frequently as required), including 6-month and annual reassessmentsAddress healthcare management issues and participate in risk assessments when neededAdvocate for consumers by collaborating with MCO care managers and case management teamsEducation & Community EngagementServe as a resource on personal care services, disability supports, and community programsConduct training sessions for staff and caregiversAssist with outreach efforts and promote awareness of consumer-directed care optionsDocumentation & ComplianceMaintain accurate, up-to-date records in compliance with GSIL policies and proceduresEnsure all documentation meets regulatory and organizational standardsAdditional ResponsibilitiesManage a caseload of PCA, NF Transition, and/or PCSP consumersAssist with Nursing Facility Transitions and ongoing monitoringSupport ACE recruitment efforts and maintain accurate requestsParticipate in committees, trainings, and professional development opportunitiesWork a flexible schedule in accordance with program and consumer needsPerform other duties that support independent living opportunitiesWhat You BringRegistered Nurse (RN) with a current New Hampshire licenseMinimum of 2 years' experience working with individuals with long-term health care needsA strong commitment to supporting independence and self-direction for individuals with disabilities or chronic health conditionsExcellent communication, organizational, and collaboration skillIf you're ready to combine your clinical expertise with a passion for advocacy and independence-while enjoying a more flexible, balanced schedule-we'd love to hear from you!Benefits:Dental insuranceFlexible spending accountHealth insuranceHealth savings accountLife insurancePaid time offProfessional development assistanceRetirement planTuition reimbursementAbout UsGranite State Independent Living is a statewide non-profit and New Hampshire's only Center for Independent Living, we stay close to our roots with a focus on our 5 core services of education, information, advocacy, support, and transition services for people with disabilities and underserved youth.
