Admissions Coordinator - Skilled Nursing

Agawam Rehab & Nursing Campus is seeking a full-time Admissions Coordinator to join our Admissions team. Working alongside experienced admissions professionals and reporting to the Regional Admissions Director, you'll help coordinate resident admissions, communicate with referral sources, and ensure a smooth, welcoming experience for residents and families across our skilled nursing campus.What You'll DoCoordinate the admissions process from referral through resident arrivalCommunicate with hospitals, referral sources, residents, families, and the interdisciplinary teamCollect and verify insurance information and admission documentationMeet with residents and families to complete the admissions processMaintain accurate records and data within facility systemsHelp ensure timely admissions and a positive experience for residents and familiesWhat We're Looking ForExperience in admissions, patient access, case management, discharge planning, healthcare coordination, or skilled nursing preferredKnowledge of post-acute care, rehabilitation, insurance verification, Medicare, or Medicaid preferredStrong communication, organizational, and customer service skillsAbility to prioritize multiple tasks in a fast-paced healthcare environmentSkilled nursing or long-term care experience is a plusCompensation & Benefits$45,000-$50,000 annually, based on experienceMedical, dental, and vision insurance401(k)Paid time off
