Licensed Clinical Social Worker - Palliative Care

Serenity Hospice Care NJ is seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our growing Palliative Care Services team.

This position is ideal for a social worker who enjoys working independently in the community and is passionate about supporting patients and families facing serious and chronic illnesses.

Travel Requirement: Approximately 85% travel throughout the assigned service area

As a Palliative Care LCSW, you will provide psychosocial support to patients and their families while helping them navigate the emotional, social, and practical challenges associated with serious illness.

This is a highly field-based position. The LCSW will spend approximately 85% of their workday traveling to patient homes and community locations, with limited time in the office.

• Provide psychosocial assessments and supportive counseling to palliative care patients and their families.

• Evaluate patients' emotional, social, and environmental needs.

• Assist patients and families with coping strategies related to serious illness, changes in health, and end-of-life concerns.

• Provide emotional support, crisis intervention, and appropriate counseling.

• Assist families with advance care planning and navigating available community resources.

• Connect patients and caregivers with appropriate community, social service, financial, and behavioral health resources.

• Collaborate closely with physicians, nurse practitioners, nurses, and other members of the interdisciplinary palliative care team.

• Participate in interdisciplinary team meetings and care planning.

• Advocate for patients' needs and preferences.

• Complete timely and accurate clinical documentation.

• Maintain compliance with all applicable professional, regulatory, and organizational standards.

• Travel extensively throughout the assigned territory to provide services in patients' homes and other appropriate settings.

• Perform other duties as assigned.

• Master's degree in Social Work (MSW) required.

• Active New Jersey LCSW license required.

• Previous experience in palliative care, hospice, home health, oncology, geriatrics, behavioral health, or a related healthcare setting strongly preferred.

• Excellent communication, interpersonal, and counseling skills.

• Ability to work independently while functioning effectively as part of an interdisciplinary team.

• Strong organizational and time-management skills.

• Ability to manage a flexible, field-based schedule.

• Valid driver's license and reliable transportation required.

• Comfortable traveling approximately 85% of the time throughout the assigned service area.

• Ability to work compassionately with patients and families experiencing complex medical and emotional circumstances.
