Job Description: (LPN) Licensed Practical Nurse

Complete Care at Berkshire is seeking a dedicated and compassionate (LPN) Licensed Practical Nurse to join our team in Reading, Pennsylvania. You will play a critical role in providing high-quality patient care in a fast-paced and dynamic environment.

About the Role: (LPN) Licensed Practical Nurse
As an LPN, you will be responsible for providing personalized care to patients in our long-term care facility, with a focus on evening shifts. You will administer medications, monitor vital signs, and report any changes or concerns to the healthcare team. You will also assist patients with daily living activities, such as bathing, dressing, and grooming. You will work closely with our interdisciplinary team to develop and implement individualized care plans for our patients.

Responsibilities: (LPN) Licensed Practical Nurse

• Provide direct patient care, including vital signs, wound care, and treatment administration
• Administer medications and treatments as ordered by physicians
• Document patient care and progress in medical records
• Assist patients with daily living activities, such as bathing, dressing, and grooming
• Report any changes or concerns to the healthcare team
