Responsibilities:

• Administer medications, treatments, and therapies as prescribed by physicians

• Monitor and document patient vital signs, conditions, and progress

• Develop and implement individualized care plans in collaboration with the multidisciplinary team

• Provide education and support to patients and families on disease management, wound care, and other health-related topics

• Participate in quality improvement initiatives to improve patient outcomes and satisfaction

• Collaborate with other healthcare professionals to ensure seamless patient care

Requirements:

• Current Registered Nurse (RN) license in the state of Pennsylvania

• Minimum 1 year of experience as a Registered Nurse (RN) in a long-term care or rehabilitation setting

• Strong clinical skills and knowledge of nursing practices and protocols
