Clinical Liaison

Candlewood Rehabilitation and Nursing Center and Atlas Healthcare are searching for a Weekend Per Diem Clinical Liaison. The individual acts as a liaison between area hospitals to promote admissions and conducts assessments of prospective admissions for appropriateness.

Responsibilities include:

• Conducting pre-admission assessments of the clinical status of a resident referral.

• Develops community and healthcare partnerships and relations as a referral source of admissions and census growth.

• Works in conjunction/coordination with the Director of Admissions, Director of Nursing and Administrator to build and maintain census at or above budget.

• Serves as liaison to hospitals, physicians, case managers, community healthcare facilities and services, families, and residents.

• Maintains knowledge of evolving long term care rules/regulations, CMS-Medicare reimbursement guidelines and participates in facility mandatory in-services.

• Produces routine and ad hoc reporting according to directives and business needs.

Qualifications:

• Registered Nurse licensed in the State of Connecticut with at least three (3) years’ experience in an acute care setting or a Licensed Practical Nurse licensed in the state of CT. Long Term Care experience preferred.

• Minimum of two (2) years marketing, admission, and/or clinical evaluator experience.

• Proficiency with the MS Office suite (e.g., outlook, work, excel).

Please Note:

• This is a Per diem role

• This is a multi-facility role

• Remote position

• 2 weekends a month

• Some holiday coverage

• May be asked to cover for full time vacations
