As a MDS Coordinator at Avir at Pearland, you will play a vital role in ensuring the highest quality of care for our residents by coordinating the development and maintenance of the Minimum Data Set (MDS) 3.0 assessment for our skilled nursing and rehabilitation center.

Responsibilities:

• Coordinate the development and maintenance of MDS 3.0 assessments for residents, ensuring accurate and timely completion to meet regulatory requirements
• Collaborate with our interdisciplinary team to gather and analyze resident data, identifying areas for improvement and developing strategies for care plan implementation
• Monitor and report on resident outcomes, submitting data to state and federal agencies as required
• Conduct unit-based assessments, identifying and addressing resident care needs, and implementing interventions as necessary
• Develop and maintain effective working relationships with our healthcare team, including physicians, nurses, and therapists
• Maintain accurate and detailed records of resident assessments, care plans, and progress

Requirements:

• 2 years of experience in a healthcare setting

• min. LVN

• degree in a healthcare-related field Required
