Social Worker at Nursing Home

Amberwoods of Farmington is seeking a Social Worker to join our team for Case Management of patients on our short-term rehabilitation unit.

Amberwoods is a 130-bed beautifully renovated, successful facility providing long-term skilled nursing, short-term rehabilitation and Memory Care services, located in Farmington, CT. Our facility is conveniently located within proximity to I-84 and is a ten-minute drive to downtown Hartford.

- Completes required EMR documentation in a timely manner including assessments.

- Completes intake for new admissions including discharge plan.

- Uses her/his resources to set up services for successful discharge after short-term rehab.

- Holds Resident Care Plan Conferences with the interdisciplinary team to review resident's plan of care.

- Assists the interdisciplinary team with any concerns that arise with residents and/or families.

- Support residents' psychosocial needs.

- Maintains timely and accurate documentation of above.

Experience:

- Nursing Home or case management experience preferred but can train the right person.

Education Requirements:

-  Bachelors Degree in Human Services or Social Work
