About the Role:

As a Home Health Caregiver, you will be responsible for providing one-on-one care to our clients in the comfort of their own homes. Your tasks may include:

• Assisting with daily living activities such as bathing, dressing, and grooming

• Managing medications and providing vital signs monitoring

• Accompanying clients to appointments and providing transportation as needed

• Maintaining a safe and clean living environment

• Providing emotional support and companionship

Responsibilities:

• Provide high-quality, patient-centered care to clients in their homes

• Maintain accurate and detailed records of client care and services provided

• Collaborate with the Modani Care team to develop personalized care plans for each client

• Promote a safe and welcoming environment for clients and their families

• Maintain confidentiality and adhere to HIPAA guidelines

Requirements:
