Job Summary:
Modani Care is seeking a compassionate and dedicated Home Health Caregiver to provide high-quality care and support to clients in their homes. Responsibilities:

• Provide high-quality care and support to clients in their homes, focusing on their physical, emotional, and social needs
• Assist with daily living activities such as bathing, dressing, grooming, and toileting
• Assist with mobility and transfers, including walkers, canes, and wheelchairs
• Provide medication reminders and assistance with medication management
• Collaborate with our interdisciplinary care team to develop and implement individualized care plans
• Promote independence, dignity, and well-being of clients, while encouraging social interaction and engagement

About the Role:
As a Home Health Caregiver with Modani Care, you will work closely with our team of healthcare professionals, including registered nurses, therapists, and social workers, to ensure that clients receive comprehensive and coordinated care. At Modani Care, we provide in-home care services for seniors and adults who need support with daily living, and our team helps families understand care options, navigate state-specific programs, and receive personalized support that allows individuals to remain safe and comfortably at home.

Salary: $14 - $14 HOURLY
