Requirements:
High school diploma, GED, or equivalent education
1+ years of recent relevant experience
Experience with healthcare operations, care coordination, or related administrative workflows
Ability to coordinate transition-of-care and discharge-related activities across multiple stakeholders
Proficiency in intermediate PC, word processing, and electronic documentation
Strong oral and written communication skills
Ability to work independently with minimal direction
Ability to prioritize competing tasks in a fast-paced environment
Ability to exercise discretion and seek input when appropriate
Preferred Requirements:
Experience with skilled nursing facility admissions or intake
Experience working in a skilled nursing facility or post-acute care setting
Experience with insurance-related workflows or payer coordination
Experience communicating with healthcare providers, facilities, families, and external vendors
Summary / Duties:
The Case Management Assistant II will support the Care Coordination department at California Pacific Medical Center’s Davies Campus. This is a non-clinical, minimally patient-facing role focused primarily on skilled nursing facility admissions and intake rather than direct patient care. Responsibilities include coordinating transition-of-care and discharge activities, gathering and organizing required information, communicating with care providers, skilled nursing facilities, families, insurance contacts, and external vendors, and helping ensure timely placement and discharge. The assistant will manage multiple priorities, maintain accurate documentation, follow established workflows, escalate questions when needed, and work independently in a fast-paced environment. Strong communication, organization, time management, computer, and word processing skills are required.
