Social Worker

Essential Duties and Responsibilities:

● Be knowledgeable of ADHC/CBAS laws and regulations to include but not limited to: CBAS

certification standards, standards of participation (SOP’s), policies, procedures, and Title

22 Regulations. This shall be met through ongoing staff training and education.

● Responsible for the implementation of the Center’s screening, intake, assessment, admission, and the discharge process. The Social Worker will follow up on all inquiries in a timely manner. Meets with family members or referring agencies and determines appropriate program.

● Provides in-home assessments for all CBAS/ADHC participants. Completes social service assessments, reassessments, and quarterlies on participants. Implements the center’s Person-Centered Care Planning Process. (Reassessments will be conducted every six months and as needed whenever a change of conditions occurs or hospitalizations or length of absence)

● As part of the care coordination intervention, the Social Worker will provide discharge planning for all participants every six months or as needed.

● Provides care coordination for CBAS/ADHC participants and their family members or authorized agents to and from the Center.

● Develop the Person-Centered Plan of Care for each participant and will document the response to each intervention on a daily basis. Will document the participant’s health record of progress or lack of progress towards goals. The Social Worker will provide services as specified at the IPC and will document services on the same day.

● Will supervise the social worker assistant and/or social work aide.

● Provides group sessions and provides encouragement to participate in group sessions.

● Provides participant and family counseling as needed.

● Enforces the implementation of participants’ rights to ensure compliance.

● Implements the grievance procedure when needed.

● Facilitates caregiver support groups as assigned.

● Provides in service training to program aides and employees when needed.

● Collaborate with the various community organizations to recommend referrals for participants being served in the program.

● Implement the Center’s community outreach plan.

● Provide referral for persons not appropriate for adult day health care.

● Initial and continuing assessment of the participant by the medical social worker in the formation of an individual plan of care by the multidisciplinary team.

● The medical social worker shall be a full-time employee of the adult day health center.

● Provide a signed and dated documentation for all services performed on the day the service is provided, and include signed and dated quarterly progress reports in each participant record.

● Provide counseling and referral to available community resources.

● Serve as liaison with other community agencies that may be providing services to a participant and work with these agencies to coordinate all services delivered to the participant to mee the participant’s needs and avoid duplication. Liaison shall include, but not be limited to, the following agencies:

(A) In-Home Support Servies in the county welfare department.

(B) Home Health Agency providers.

● A medical social worker shall be a full-time employee of the adult day health center.

● A medical social worker shall act as a member of the assessment team to evaluate the medical social needs of all participants. The medical social worker shall develop a plan of care if indicated, including short-term and long-term goals with participation of the participant’s family and other appropriate individuals.

● The job description is in no way meant to describe exactly what the job duties are. It is a guideline to provide a frame of reference for job requirements. Other duties may be assigned at the discretion of management.

● Create a team approach in providing services.

Education and/or Work Experience Requirements:

The Social Worker shall be a person who meets one of the following:

● (a) Is licensed as a clinical social worker by the Board of Behavioral Sciences; or

● (b) Holds a Master of Social Work degree from a school of social work accredited by the Council on Social Work Education; or

● (c) Holds a master’s degree in psychology, gerontology, or counseling from an accredited school, and has one year of experience providing social services in one or more of the fields of aging, health, or long-term care services; or

● (d) Holds a bachelor’s degree in social work from an accredited school with two years of experience providing social services in one or more of the fields of aging, health or long-term care services.
