Position Summary:

  • Provide occupational therapy interventions under the supervision of a licensed Occupational Therapist and Director of Rehabilitation Services.
  • Help residents maintain or regain daily living, adaptive, and functional skills.
  • Implement established occupational therapy plans through ADL training, purposeful activities, therapeutic exercises, and adaptive techniques.
  • Support upper-extremity function, fine-motor skills, cognition, safety awareness, positioning, and functional independence.
  • Assist with wheelchair/seating, power mobility, hand orthotics, adaptive equipment, and discharge-related programs.
  • Educate residents and caregivers on assigned strategies, equipment, and safe performance of daily tasks.
  • Monitor resident response and report changes, concerns, and progress to the supervising Occupational Therapist.
  • Maintain treatment areas, sanitize equipment, and complete accurate treatment and progress documentation.

Requirements:

  • Current, unrestricted New York State Occupational Therapy Assistant license.
  • Minimum 1 year of supervised Occupational Therapy Assistant experience.
  • Skilled nursing, long-term care, geriatric, rehabilitation, ADL, wheelchair/seating, or memory-care experience preferred.
  • Facility credentialing required, including proof of license/credentials, current physical examination, and TB/PPD documentation.
  • Maintain resident confidentiality and complete timely, accurate documentation in the facility EHR.
  • Ability to work effectively with nursing, medical, social services, residents, families, VA, and outside equipment vendors.
