Speech-Language Pathologist (SLP) – Job Description
  • Position: Speech-Language Pathologist
  • Location: Montrose, New York 10548
  • Employment: Full-time, onsite, W-2
  • Facility: 252-bed skilled nursing & rehabilitation facility
  • Department: Rehabilitation / Therapy
  • Hours: 40 hours/week
  • Schedule: Within 7:00 AM–4:00 PM; rotating weekends/holidays
  • Target Start: September 2026
  • Pay: $58–$62/hour base pay

Key Responsibilities

  • Evaluate, test, and treat speech, voice, language, cognitive-communication, and swallowing disorders.
  • Develop individualized treatment plans and provide rehabilitation, counseling, consultation, and resident/family education.
  • Screen new admissions for diet and swallowing safety.
  • Recommend appropriate diets, positioning, compensatory strategies, and adaptive tools.
  • Participate in care-plan and discharge-planning meetings.
  • Coordinate with nursing, dietary, medical providers, residents, and families.
  • Maintain timely and accurate clinical documentation.

Minimum Qualifications

  • Current New York State SLP or Audiologist license appropriate to the assigned scope.
  • Minimum 3 years of supervised speech therapy experience.
  • Skilled nursing, geriatric, dysphagia, cognitive-communication, memory-care, or long-term-care experience preferred.
  • Complete facility credentialing, including license/credentials, physical exam, and TB/PPD documentation.
  • Ability to maintain confidentiality and complete documentation in the electronic health record.
  • Strong collaboration skills with nursing, medical, social services, residents, families, VA, and outside vendors.
