- Schedule: Within 7:00 AM–4:00 PM; rotating weekends/holidays
- Target Start: September 2026
- Pay: $58–$62/hour base pay
Key Responsibilities
- Evaluate, test, and treat speech, voice, language, cognitive-communication, and swallowing disorders.
- Develop individualized treatment plans and provide rehabilitation, counseling, consultation, and resident/family education.
- Screen new admissions for diet and swallowing safety.
- Recommend appropriate diets, positioning, compensatory strategies, and adaptive tools.
- Participate in care-plan and discharge-planning meetings.
- Coordinate with nursing, dietary, medical providers, residents, and families.
- Maintain timely and accurate clinical documentation.
Minimum Qualifications
- Current New York State SLP or Audiologist license appropriate to the assigned scope.
- Minimum 3 years of supervised speech therapy experience.
- Skilled nursing, geriatric, dysphagia, cognitive-communication, memory-care, or long-term-care experience preferred.
- Complete facility credentialing, including license/credentials, physical exam, and TB/PPD documentation.
- Ability to maintain confidentiality and complete documentation in the electronic health record.
- Strong collaboration skills with nursing, medical, social services, residents, families, VA, and outside vendors.
