• Minimum 1 year of Home Health Physical Therapy experience
• HomeCare HomeBase (HCHB) EMR experience required
• OASIS-E assessment experience required
• Experience with:
  • Chronic Disease Management
  • Orthopedic Rehabilitation
  • Total Hip and Knee Replacement Recovery
  • Fall Prevention
  • Balance Training
  • Gait Training
  • CHF Management
  • COPD Management
  • Stroke Rehabilitation
  • Geriatric Patient Care

Preferred Experience

• Experienced Home Health traveler
• Strong care coordination and communication skills
• Ability to work independently in the field
