Description
American Traveler is seeking a Home Health Physical Therapist with at least 1 year of skilled home health experience for a 13-week assignment in the Medford, OR area.
Details
- Home health setting covering Medford, OR and surrounding areas
- Patient ratio is 1:1 with up to 6 patients visited per day
- Standard productivity requirement of 30 points
- Day shift schedule, 5x8 hours (8:00 AM - 4:30 PM)
- No weekend or on-call requirements
- No floating between units required
- Uses Epic and Oasis for charting
Requirements
- Minimum 1 year of skilled home health experience required
- Current BLS certification required
- Oasis experience required
- Must have own reliable transportation for home visits
- License required if applicable
- Primary series COVID vaccination plus one booster required
Additional Information
- Responsibilities include home exercise program development and patient/family education
- Occasional travel of 50+ miles between patient visits, though scheduling is optimized to minimize drive time
- Scrubs of any color accepted
- Free parking provided along with IRS standard mileage reimbursement
- First-time travelers will not be considered for this position
- Local candidates within a 50-75 mile radius accepted at the same pay rate
- Former Providence employees may return as travelers after a 3-month waiting period
- Facility can cancel up to one shift per pay period
