Job Summary:
Physical Therapist (PT) specializing in Home Health (HH) services in Medford OR and surrounding areas. This position involves providing skilled physical therapy care to patients in a home health setting. The role requires adherence to clinical protocols and delivering personalized therapy to promote patient recovery and mobility.
Job Details:
- Schedule: 8-hour days
- Required Qualifications: 1 year of skilled home health experience, valid state license (compact license accepted), BCLS certification, and at least 1 year of professional experience.
- Preferred Qualifications: Home health experience is preferred.
- Float: Float rotation may be required.
- Additional Information:
- Traveler is expected to have their own transportation; occasional maximum radius of 50+ miles.
- COVID-19 vaccination (primary series) and one booster are required.
- Must commit to working 3 of the 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day); all scheduling requests must be made upfront.
- Call required: No.
- System used for charting: EPIC/Oasis.

- Attire: Any color scrubs.
