Job Summary:
Physical Therapist (PT) specialized in Home Health (HH) services is sought for a facility in Medford, Oregon, and surrounding areas. This position involves providing skilled home health therapy with a focus on patient-centered care and functional outcomes. The role is scheduled for dayshift hours, with a start date of September 8, 2026, and a projected end date of December 8, 2026.

Job Details:

• Location: Medford, OR, and surrounding areas
• Schedule: 8-hour days, minimum 40 hours per week
• Required Qualifications: Valid state license (compact license accepted), BLS certification, at least 1 year of skilled home health experience, and proficiency in charting systems (any system acceptable).
• Preferred Qualifications: Home health experience preferred; previous experience with EPIC/Oasis charting system is desirable.
• Certifications: BLS required
• Special Requirements: Traveler must have their own transportation; occasional drive radius up to 50+ miles; COVID-19 vaccination (primary series plus one booster) required; Oasis certification required.
• Additional Information:
  • Must commit to working 3 of the 5 major holidays.
  • RTO requests should be specified during submission; additional RTO may delay approval.
  • Candidate should have at least 1 year of home health experience, with no experience required otherwise.
  • No float requirement, and local candidates within 50 miles are considered.
  • References: 2 references covering at least 1 year in the last 3 years required.
  • Travel compliance and policy restrictions apply, including no Nursys or background check hits accepted.
  • Candidate responses regarding prior employment at Providence facilities and references are required upon submission.
