Job Opportunity – Physical Therapist, Home Health – Medford, OR 97504

Start Date: As soon as possible

Location: Medford, Oregon and surrounding areas

Schedule: Day shift, 8-hour shifts (08:00 - 16:30), Monday through Friday. This position follows a standard 40-hour work week with 1:1 patient ratios. Therapists may visit up to 6 patients per day with a standard productivity expectation of 30 points.

Requirements:

• Active Oregon Physical Therapy license or compact license (must be in hand at time of submission)
• Minimum of 1 year of skilled home health experience required
• Current BLS (Basic Life Support) certification
• OASIS documentation experience required
• COVID-19 primary vaccination series plus one booster required
• Reliable personal transportation necessary for home visits
• Two professional references covering 1 year within the last 3 years
