Social Worker (LBSW) / Care Manager - Case Management

We are seeking professionals to join a Case Management team focused on coordinating patient care, supporting safe transitions, and ensuring patients receive appropriate and timely services.

Pay: $24.77+/hour for BSW Social Worker | $30.11+/hour for Care Manager
Type: Full-Time, Permanent
Schedule: Day Shift | 8-hour shifts
Openings: 3
Relocation: Up to $5,000
On-Call: Possible

Key Responsibilities

• Assess patient and family needs and develop appropriate care plans.
• Coordinate services and resources across the healthcare team.
• Monitor patient progress and evaluate ongoing care needs.
• Support quality, safety, and timely delivery of services.
• Communicate with patients, families, providers, and multidisciplinary teams.

Requirements

For Social Worker (BSW/LBSW):

• Bachelor’s Degree in Social Work/Social Services.
• Hospital-based social work experience preferred.

For Care Manager:

• Associate’s Degree or Diploma from an accredited RN program.
• Current Kansas RN license.
• 2 years of acute-care Med/Surg nursing experience required.
• BSN preferred.
