Summary:
We are currently seeking a compassionate and organized LPN (MC) Assistant Case Manager to join our team. This position will provide administrative, documentation, coordination, and Licensed Practical Nurse support in collaboration with the Memory Care Director and Case Manager.
The LPN Memory Care Case Manager Assistant will assist with assigned LPN and case management responsibilities and help ensure that resident needs, documentation, communication, and follow-up activities are addressed in a timely and organized manner. This is an excellent opportunity for an LPN who enjoys working closely with residents, families, and an interdisciplinary team while supporting the overall case management process for our memory care community.
Responsibilities:
Provide administrative and case management support to the Memory Care department
Assist with assigned case management tasks and follow-up activities for residents.
Maintain accurate, timely, and organized resident documentation and records.
Assist with gathering and organizing information needed for resident assessments, and care planning.
Help coordinate communication and follow-up with residents, families, and members of the care team as directed.
Support the Case Manager and Memory Care Director in monitoring resident needs and ensuring appropriate follow-up.
Assist with maintaining documentation and records in accordance with company policies and applicable regulatory requirements.
Collaborate with team members to promote resident well-being, dignity, rights, and quality of life.
Maintain confidentiality and protect resident privacy in accordance with company policies and applicable regulations.
Perform other related duties and assignments as directed by the Memory Care Director.
Education & Experience:
Active LPN license in good standing, required.
Prior experience in senior living, assisted living, memory care, healthcare, or a related setting, required.
Strong organizational and administrative skills with attention to detail.
Ability to communicate effectively and compassionately with residents, families, and members of the care team.
Ability to maintain confidentiality and handle sensitive resident information appropriately.
Comfortable working independently on assigned tasks while collaborating closely with the Memory Care Director
