Coordinate the administration of a comprehensive functional assessment by the interdisciplinary team according to OBRA /F 406) IDPA specialized regulations. Determine and document the discharge potential of each resident upon admission. Coordinate the development of comprehensive Program Plan (CPP) and participate in the establishment of priority treatment goals and programs. Update assessments quarterly by the IDT. Assist resident. coordinate the goals & service of all providers of mental services to the resident Review and document resident's response to the CPP and progress toward goals.
