Physical Therapist (PT), Home Health, PRN, 1099
Selectively hiring for PRN Home Health Physical Therapist (PT). We are an elite home health therapy team. Essential Functions & Responsibilities of the Physical Therapist (PT): 

• Perform comprehensive assessments and evaluations of patient care needs, including: functional status and mobility, endurance and stamina, pain levels and other medical conditions
• Develop and implement individualized treatment plans as a Physical Therapist (PT)
• Provide skilled interventions to improve patient well-being and functional outcomes
• Monitor and evaluate patient progress, adjusting care plans as needed
• Deliver thorough education and training to patients and caregivers
• Participate in staff and administrative meetings

Qualifications for the Physical Therapist (PT)

• Active license from the Florida Board of Physical Therapy
• Valid Florida driver’s license, active vehicle insurance, and reliable transportation
• Ability to pass AHCA Clearinghouse background screening (https://info.flclearinghouse.com)
• Current in-person CPR certification
• Physical ability to perform job duties including standing, bending, lifting, and assisting patients/equipment
• Strong customer service, communication skills and effective listening
• Efficient and timely documentation

Experience Preferred for the Physical Therapist (PT)

• Minimum 1 year of Physical Therapy experience
• Minimum 1 year of Home Health experience

Compensation for the Physical Therapist (PT)

• Pay is commensurate with Physical Therapist (PT) experience, Home Health background, and documentation proficiency
• Pay is based on per visit type
