ComForCare Home Health Care - Baltimore/Carroll is looking for a compassionate and skilled Registered Nurse (RN) to join our growing home care team in Towson, MD. Responsibilities:

• Conduct comprehensive patient assessments and develop individualized care plans
• Administer medications, treatments, and skilled nursing interventions in the home setting
• Monitor and document patient progress, vital signs, and changes in condition
• Coordinate care with physicians, therapists, and other healthcare team members
• Educate patients and family members on disease management, medications, and home safety
• Ensure accurate and timely clinical documentation in compliance with regulatory standards

Requirements:

• Active Registered Nurse (RN) license in the state of Maryland
• Minimum 2 years of clinical nursing experience; home health experience preferred
• Strong assessment, critical thinking, and clinical decision-making skills
• Excellent communication and patient education abilities
• Reliable transportation and a valid driver's license
• Proficiency with electronic health records (EHR) and clinical documentation
• CPR/BLS certification required
