Director of Coordination - Home Care

We are seeking an experienced Director of Coordination to oversee and lead a large Scheduling/Coordination Department within a high-volume home care agency serving approximately 6,000 patients.

This is a senior leadership position for someone who has grown within home care coordination—from an experienced Scheduling Coordinator into a Team Lead, Manager, or other leadership role within a Coordination Department.

Schedule: Full-Time, Monday-Friday, 9-5

Key Responsibilities

• Oversee the day-to-day operations of a large team of Scheduling Coordinators
• Manage and provide direction to Team Leads/Managers who report to the Director of Coordination
• Delegate responsibilities effectively across teams and ensure accountability
• Ensure patient cases are appropriately staffed, and scheduling needs are handled efficiently
• Monitor department productivity, performance, attendance, and workflow
• Identify staffing or scheduling issues and ensure they are escalated and resolved quickly
• Develop and maintain organized processes for a high-volume coordination floor
• Coach, mentor, and develop Team Leads and Coordinators
• Work closely with senior leadership and other departments to ensure smooth agency operations

Qualifications

• 5+ years of home care Scheduling/Coordination experience required
• Must have progressed into a Team Lead, Team Manager, Coordination Manager, or similar leadership position
• Strong understanding of home care scheduling and coordination operations
• Proven ability to manage large teams in a fast-paced, high-volume environment
• Excellent leadership, delegation, organization, and problem-solving skills
• Ability to hold Team Leads and staff accountable while maintaining a productive team environment
• HHAeXchange experience is required
• Bilingual preferred
