Description
American Traveler seeks a Speech Language Pathologist for a Home Health position requiring 1+ year of Home Health experience and HCHB EMR proficiency.
Details
- Home Health setting providing care to patients throughout their homes in the community
- Travel throughout Albemarle, Badin, Harrisburg, Mt Gilead, Norwood, Troy and surrounding areas
- Treats adult patients with cognitive impairments, tracheostomy care needs, dysphagia, stroke, and aphasia
- Schedule requires 6 patient visits per day with 30 visit equivalents (VE) required weekly
- Average daily driving distance of approximately 60-80 miles, with mileage reimbursed weekly
- 8-hour day shift schedule, 0800-1700, 40 standard hours per week
- Uses EPIC Rover (view only), Mosaiq for uploading discharge instructions and wound documentation, and Homecare Homebase EMR
- Company-provided phone used for documentation and communicating with patients and providers
- No weekend rotation, no call requirements, and no major holiday coverage
- Floating required within Stanly and Montgomery counties
- Schedules visits with patients one day prior to the planned appointment
Requirements
- Active Speech Language Pathology license required if applicable
- Minimum 1 year of Home Health experience required
- Valid driver's license and proof of insurance required
- Homecare Homebase (HCHB) EMR experience required with no exceptions
- Self-motivated with ability to work independently
- Strong time management skills
- Ability to communicate effectively with a multi-disciplinary team
- Last 4 digits of SSN and full date of birth required for consideration
- Minimum 1 professional reference dated within the last year from a Supervisor, Manager, Director, or Charge RN

- Candidates must reside more than 50 miles from the facility to be eligible for the travel position
