Job Summary
Occupational Therapist Home Health (OT HH) Location unspecified. This position involves providing skilled occupational therapy services to adult patients in a home health setting. The role requires delivering personalized care to meet individual patient needs and maintaining timely documentation.
Job Details
- Certifications: BLS, NBCOT
- Education Requirements: Must possess a valid Washington state license or a compact license
- Experience Requirements: Minimum of 1 year of recent Skilled Occupational Therapy experience (no inpatient experience accepted), with at least 1 year within the last 3 years. OASIS experience required for staff completing Start of Care; proficiency in Homecare Homebase (HCHB) required.
- Schedule: 8-hour shifts, Days, minimum of 40 hours per week.
- Responsibilities include delivering OT services to adult patients, managing 4-5 patients daily, and ensuring documentation is completed accurately and on time.
- Additional Notes: The traveler is expected to have reliable transportation, with occasional maximum radius of 100 miles due to territory. Timely communication and documentation are mandatory.
- Submission Requirements: Active drivers license, personal transportation, and Monday orientation. RTO (Requested Time Off) must be submitted at the time of application. On-call rotation may be required.
