This home health nurse traveler role is based in Lombard, Illinois, a vibrant and welcoming suburb located just west of Chicago. Lombard offers a comfortable, classic midwestern lifestyle with tree-lined streets, well-kept neighborhoods and a friendly, community-centered atmosphere. The area features convenient access to shopping, dining and entertainment, including the popular Yorktown Center, local cafés and restaurants, and nearby parks and recreational paths that make time off enjoyable and relaxing. Lombard is well-connected to the broader Chicago region, with easy access to major highways and public transportation, making commuting between patient homes efficient and straightforward. The community is known for its safe residential areas, seasonal local events and a balance of urban amenities with suburban charm, which travelers often find appealing for both work and leisure. In this role, the home health nurse traveler will provide direct nursing care to patients in their homes across Lombard and surrounding western suburbs. Typical responsibilities include performing head-to-toe assessments, managing medications, providing wound care, monitoring vital signs and symptoms, and supporting chronic disease management for adult and geriatric patients. The nurse will deliver patient and family education focused on safety, disease understanding, home treatment plans and prevention of hospital readmissions. A typical day involves traveling to multiple home visits, documenting care and assessments in an EMR system, collaborating with RNs and LPNs and other members of the interdisciplinary team, and communicating changes in patient status promptly. Visit volumes and daily caseloads are managed to support safe practice and quality care, with scheduling designed to allow adequate time for meaningful patient interaction and thorough documentation. The work environment is field-based, requiring comfort with autonomous practice and strong organizational skills to manage routes, supplies and time. Patient acuity ranges from stable chronic conditions requiring routine monitoring to more complex post-acute cases recently discharged from hospital or rehab settings. The nurse traveler will have opportunities to build skills in home-based care, care coordination and teaching, all within a supportive and collaborative home health culture that values clinical excellence and patient-centered service.
