What You'll Do

As a Case Manager, you'll work directly with children, families, resource families, and community partners to support the needs and goals of the children we serve. Your work will include:

  • Building positive, supportive relationships with children and families.
  • Advocating for children and helping ensure their needs are addressed.
  • Coordinating services and resources, including medical, educational, behavioral, recreational, and community supports.
  • Developing and monitoring plans related to family visits, reunification, placement, and permanency.
  • Communicating with families, schools, service providers, referring agencies, and other professionals.
  • Attending court hearings, meetings, trainings, conferences, and other required activities.
  • Coordinating transportation and, when appropriate, transporting children and families.
  • Completing timely, accurate, and comprehensive documentation and reports.
  • Responding to challenges with creativity, problem-solving, and resourcefulness.

This Is a Community-Based Position

Travel is an essential part of this role. Case Managers spend significant time outside of the agency office and regularly travel throughout their assigned service area to meet the needs of children and families.

Travel may include:

  • Private residences
  • Foster and adoptive homes
  • Schools and educational settings
  • Community locations
  • Provider agencies and service locations
  • Courts and other legal proceedings
  • Other locations necessary to support children and families

You must be able to independently travel to required locations and maintain reliable transportation for work-related travel. The position involves frequent face-to-face contact with children, families, resource families, providers, and other professionals.

Because the needs of children and families vary, the work schedule may also vary. You may need to adjust your schedule to attend family visits, court hearings, meetings, assessments, appointments, and other activities necessary to support the children and families we serve.

This is not a primarily office-based position. Successful candidates should be comfortable traveling regularly, working independently in the community, managing multiple appointments and locations, and working in a variety of environments.

What We're Looking For

  • Bachelor's degree preferred, particularly in social work or a related field.
  • Two years of experience working with children and/or families preferred.
  • Ability to meet current Pennsylvania legal and regulatory requirements.
  • Valid Pennsylvania driver's license.
  • Ability to obtain required PA Child Abuse, PA State Police, and PA FBI clearances.
  • Strong communication, organization, problem-solving, and interpersonal skills.
  • Ability to work independently while also being an effective member of a team.
  • Compassion and respect for the diverse cultural and socioeconomic backgrounds of the children and families we serve.

Physical Requirements

This position requires the ability to sit for extended periods, as well as stand, walk, bend, and reach throughout the workday. Employees must be able to lift and carry up to 10–15 pounds and operate standard office equipment and technology. Clear communication in person, by phone, and electronically is required.
