EI Service Coordinator Position Summary

The Initial Service Coordinator (ISC) assists families in navigating the Early Intervention Program (EIP) referral and evaluation process from referral through the completion of the Initial Individualized Family Service Plan (IFSP) meeting or case closure. The ISC ensures that families receive appropriate services, supports, procedural safeguards, and information regarding their rights and responsibilities under the Early Intervention Program.

Required EI Service Coordinator Qualifications

Candidates must meet one of the following New York State Early Intervention Program (EIP) service coordination qualifications:

    Two (2) years of experience providing service coordination; orOne (1) year of service coordination experience and one (1) year of experience working with infants and toddlers with developmental delays or disabilities; orOne (1) year of service coordination experience and an Associate Degree in Health or Human Services; orA Bachelor's Degree in Health or Human Services.

Additional Requirements:

    Must successfully complete the New York State Introduction to Service Coordination training course prior to employment and provide proof of completion.Must provide and maintain annual a medical clearance statement from a healthcare provider, negative TB test and symptom questionnaire thereafter, MMR vaccine or immunity, and have recommended or documented refusal of Heb B, tetanus, diphtheria, pertussis, varicella and Influenza.Must review and adhere to New York State Early Intervention Service Coordination recommended practices

Essential Duties and Responsibilities

    Serve as the primary point of contact for families referred to the Early Intervention Program.Guide families through the evaluation and eligibility process, ensuring they understand their rights, responsibilities, and available resources.Coordinate referrals, evaluations, and Initial IFSP meetings within required program timelines.Collaborate with families, evaluators, providers, and the Early Intervention Official Designee (EIOD) to ensure timely service delivery.Maintain accurate case records, documentation, and billing in compliance with agency and NYS Early Intervention requirements.Assist families with transition planning to preschool special education services (CPSE), when appropriate.Provide advocacy, information, and referrals to community resources and supports.Participate in required meetings, supervision, and professional development activities.Utilize agency-approved communication systems to respond to inquiries from families and stakeholders.
