Requirements:
- Valid and unrestricted Physical Therapist license in Oregon
- Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program
- Current BLS certification through American Heart Association (AHA)
- Completed skills checklist within one year for Physical Therapy specialty
- Immunization compliance including MMR, Varicella, Hepatitis B, TB testing, and seasonal influenza
- Negative drug screen and cleared background check
- Completion of Oregon-specific mandatory training: Providing Inclusive Care Training for Long-Term Care Settings
