Requirements:
- Valid and unrestricted Physical Therapist license in Oregon
- Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program
- Active BLS certification issued through American Heart Association (AHA) or American Red Cross (ARC)
- Completion of Oregon-specific mandatory training: Providing Inclusive Care Training for Oregon Long-Term Care Settings
- Current immunizations including MMR, Varicella, Hepatitis B, TB testing, and seasonal influenza
- Successful completion of background checks, drug screening, and competency examination
- Strong interpersonal and communication skills with commitment to patient-centered care
