Per Diem Director of Nursing (DON) / Registered Nurse (RN)

Holy Trinity Home Care Agency is seeking an experienced, dependable, and compassionate Registered Nurse to serve as our per diem Director of Nursing. We are looking for someone who believes in our mission and is willing to grow with the company as we expand our client base.

Compensation:

• $40.00–$45.00 per hour for administrative and nursing duties
• $100.00 per initial client assessment
• $75.00 per client reassessment

Responsibilities:

• Complete initial nursing assessments and reassessments
• Develop and update individualized plans of care
• Perform supervisory visits and monitor the quality of services
• Oversee CHHAs and ensure they follow each client’s plan of care
• Review client documentation and caregiver notes
• Provide caregiver education, orientation, and ongoing training
• Communicate with clients, families, caregivers, and agency management
• Report and document changes in clients’ conditions
• Assist with incident reviews and quality-improvement activities
• Help maintain compliance with New Jersey regulations, agency policies, and accreditation requirements
• Provide clinical guidance to agency staff as needed

Qualifications:

• Active and unrestricted New Jersey Registered Nurse license
• Previous home care, home health, geriatric, or supervisory experience preferred
• Strong assessment, documentation, and communication skills
• Knowledge of care planning and caregiver supervision
• Dependable, organized, and able to work independently
• Willingness to grow into a larger role as the agency expands
• Must meet all employment and background-check requirements
