Schedule: Monday-Friday, 8:00 AM to 5:00 PM with a rotating on-call schedule.

What You'll Do

As an RN Case Manager, you will serve as the primary coordinator of care for your patients, ensuring they receive the compassionate, personalized support they deserve throughout their hospice journey.

You'll:

  • Manage a patient caseload and coordinate all aspects of care planning and delivery
  • Complete comprehensive assessments addressing physical, emotional, social, and spiritual needs
  • Collaborate closely with physicians and the interdisciplinary care team to provide exceptional patient-centered care
  • Conduct routine visits and adjust care plans as patient needs evolve
  • Guide and support patients and families through the hospice experience
  • Maintain accurate and timely clinical documentation
  • Supervise Home Health Aides and ensure quality care delivery in accordance with regulatory guidelines
  • Provide compassionate end-of-life care and family support

Qualifications

  • Graduate of an accredited Registered Nursing program
  • Current RN license in the state of practice
  • Current CPR and First Aid certification
  • Knowledge of quality assessment and assurance procedures
  • Valid driver's license and proof of insurance
  • Excellent communication, assessment, and organizational skills
  • Passion for delivering compassionate, patient-centered hospice care
