EI Service Coordinator Position Summary

The Ongoing Service Coordinator (OSC) supports children and families enrolled in the New York State Early Intervention Program (EIP) following the completion of the Initial Individualized Family Service Plan (IFSP). The OSC serves as the family's primary point of contact throughout their participation in Early Intervention and ensures that services are implemented in accordance with the IFSP while protecting the family's rights and access to supports, resources, and procedural safeguards under the Early Intervention Program.

Required EI Service Coordinator Qualifications

Candidates must meet one of the following New York State Early Intervention Program (EIP) service coordination qualifications:

  • Two (2) years of experience providing service coordination; or
  • One (1) year of service coordination experience and one (1) year of experience working with infants and toddlers with developmental delays or disabilities; or
  • One (1) year of service coordination experience and an Associate Degree in Health or Human Services; or
  • A Bachelor's Degree in Health or Human Services.

Additional Requirements:

  • Must successfully complete the New York State Introduction to Service Coordination training course prior to employment and provide proof of completion.
  • Must provide and maintain annual a medical clearance statement from a healthcare provider, negative TB test and symptom questionnaire thereafter, MMR vaccine or immunity, and have recommended or documented refusal of Heb B, tetanus, diphtheria, pertussis, varicella and Influenza.
  • Must review and adhere to New York State Early Intervention Service Coordination recommended practices

Essential Duties and Responsibilities

  • Serve as the primary point of contact for families enrolled in the Early Intervention Program.
  • Build supportive relationships with families and help them navigate services, resources, and their rights within the program.
  • Coordinate and monitor Early Intervention services to ensure timely implementation of the IFSP.
  • Collaborate with families, providers, municipalities, and community partners to support positive outcomes for children.
  • Coordinate six-month and annual IFSP reviews and assist with service changes as needed.
  • Support families through the transition to Preschool Special Education (CPSE), including required.
