Responsibilities:

• Plan, develop, and implement individualized physical therapy treatment plans for patients with a variety of physical disabilities and illnesses

• Conduct thorough evaluations, assessments, and initial and follow-up treatment sessions

• Develop and implement individualized therapeutic exercises and treatment programs to improve strength, flexibility, mobility, and functional capacity

• Collaborate with healthcare teams to ensure patient-centered care and achieve optimal outcomes

• Maintain accurate and detailed records of patient treatment and progress

• Participate in multidisciplinary team meetings to discuss patient treatment and progress

Requirements:

• Current physical therapy licensure in the state of Vermont

• Current CPR certification
